ABSTRACT

Objective:To analyze the reform actions on raising the appropriateness of inpatient care use, as well as the current situation, so as to provide suggestions on improving the appropriateness.Methods:Policies and literatures on the appropriateness of inpatient care use released from 2009 to 2021 were collected from such official websites as the State Council and China National Health Commission as well as literature databases from home and abroad, for a text analysis. Based on the data of China Health Statistics Yearbook(2010-2021) and The Sixth National Health Service Survey Report (2018), descriptive methods were adopted to analyze the situation of inpatient care use in China. Results:Since the new healthcare reform, the Chinese government had standardized the hospitalization standards and procedures to minimize inappropriate use of inpatient care, increased financial subsidies for public hospitals to minimize their inappropriate patient attraction merely for economic interests, and reformed the medical insurance payment methods so as to regulate physicians′ behavior. Under the influence of the above policies, the average length of stay decreased from 10.5 days in 2009 to 9.1 days in 2019, with an average annual growth rate of -1.42%. The average waiting time in hospitals decreased from 3.6 days in 2008 to 1.5 days in 2018.From 2012 to 2019, the admission rate increased from 13.2% to 19.0%.Conclusions:Since the new healthcare reform, the average length of stay in China has decreased year by year, but the admission rate has increased year by year. Therefore, it is necessary to further reduce the inappropriate utilization of inpatient care use by speeding up the construction of the close-type medical alliances, improving the reform of payment methods, refining the performance appraisal standards for medical staff and strengthening supervision mechanism.

ABSTRACT

Since the implementation of the new healthcare reform,it has achieved some success.However,many problems still exist in the process of primary medical reform,and there are many ethical dilemmas.The un-balanced allocation of medical resources reflects the relationship between government and market,the difficult pro-motion of " three medicine" reflects the sectoral conflicts of interest,the difficult implementation of the grading di-agnosis and treatment system reflects the different demands of stakeholders in health care system.As a result,the whole function of China's medical and health system has not been fully played.Based on the ethical perspective,this paper put forward that taking the scientific definition of government and market responsibility as a framework,it should construct an investment system of health resource allocation taking equity as a value orientation; taking ethi-cal responsibility as the framework,it should promote the cooperation consensus of the linkage reform of medical treatment,medical insurance and medical departments; and taking ethical community as the constraint,it should balance the interests of all parties and promote the implementation of the policy of grading treatment system.

Returning to its commonweal feature and enhancing impartiality are the focal purposes of Chinese new healthcare reform in the near future.Rawl's theory of justice could explain the past difficulties and indicate the intending complications in the forthcoming innovation.We can see the selfishness of human nature from Rawl's Veil of Ignorance.People's greed will bring about such deadweight to national finance that the goal of the reform has to be inclined to efficiency,which forms again a seasonal movement.

ABSTRACT

The new healthcare reform project raised the thought that basic medical care service should be available to each individual,and the objective of an entire-people covered medical care assurance,which provide policy guarantee for the improvement of medical care service.To achieve the real advancement of medical care service,many endeavors are involved as follows.It is necessary to strengthen the responsibility share of all-level governments,guarantee the input for medical care adapted to local conditions and meanwhile embody medical care impartiality,strengthen the quality construction within medical care institutions,develop the role of medical care service in directing the patients triage which guides residents to get access to community medical centers,and respect individual's own choice for medical care so as to achieve the competition mechanism among medical care service designated hospitals.

ABSTRACT

The nature of medical science,especially the highly socialized characteristic of modern healthcare service determines that a larger ratio of medical ethics should be included in the new healthcare reform,with the construction of a new medical ethics taken as support and guidance.An overall view of medical ethics is crucial in the construction of medical ethics under the context of current Chinese healthcare reform,that is,the construction of medical ethics should be performed in an ultra-wide field.Medical ethics should take a complete participation in each and every field of healthcare reform,but not merely focuses on the particular field of medical professionals and the construction of a harmonious physician-patient relationship.In the systematic construction of new medical ethics,the current key issue lies in the reconstruction of a socialist medical ethics and related regulations,both for theoretical and practical reasons.The reconstruction is far more than releasing a couple of policies.Merely in a theoretical perspective,seven major issues are to be resolved during the reconstruction process.

The medical humanistic spirit is an ethical concept which takes people-oriented,life-loving,and a sustainable development of human physical and mental health as its ideal value.However,since the modern society is filled with technological doctrines,utilitarianism and market-oriented behaviors,making the humanistic spirit in medical education weakened,resulting in unethical action of healthcare services,disordered healthcare services,and the imbalance between physicians and patients.This paper studies the question of modern medicine brought about by the loss of humanistic spirit,discusses the "humanistic" feature of new healthcare reform,and bring forward a medical personnel training mode which stresses humanistic spirit.
